Abstract

While the costs to Medicare of solid organ transplants are varied and considerable, the total Medicare expenditure of $4.4 billion for solid organ transplant recipients in 2014 remained less than 1% of all Medicare expenditures. For patients covered by Medicare, the ratio of pre- to posttransplant cost of care varied widely by organ and within some organ categories by patient characteristics. This chapter reports pretransplant costs for all solid organ candidates covered by Medicare to allow investigators to further explore the relative cost of transplant compared with alternative management.

Kidney

Medicare coverage for some or all of the care of kidney transplant recipients in 2014 varied from 58.0% for children to 98.2% for recipients aged 65 years or older (Figure Econ 1). From transplant through the first year posttransplant, the average reimbursement for kidney recipients with primary Medicare coverage in 2008-2013 was $64 thousand for Part A and $19 thousand for Part B (Table Econ 2). The cost of care in the second year was approximately one-third of first-year costs, at $14 thousand for Part A and $10 thousand for Part B, totaling $24 thousand in 2008-2012 (Table Econ 3). This compares with Medicare expenditure of $47 thousand during the year preceding transplant, 194% of second-year posttransplant costs (Table Econ 1). Kidney transplant recipients experienced the least frequent rehospitalization rates among solid organ recipients, with 151 rehospitalizations per person-year at risk during the first posttransplant year and 71 during the second (Figure Econ 2). Transplant complications were the most common reason for readmission during the first posttransplant year and infections during the second. The total Part A and Part B Medicare expenditure for kidney transplant recipients was $2.5 billion in 2012 for the care of 111,531 patients; this increased to $2.6 billion in 2014 for the care of 116,939 patients (Table Econ 4). Kidney transplant recipients accounted for 65.8% of all solid organ recipients covered by Medicare and for 59.7% of all Medicare expenditures for solid organ transplant recipients in 2014. Average Medicare expenditure in 2014 for 116,939 kidney recipients was $24,879, the least expensive per patient of all solid organ transplants. The proportion of expenditure declined to 59.9% in 2013, from 60.2% in 2012 and 60.4% in 2011. The continuing decline has been due to relatively increasing coverage of recipients of other organs.

Pancreas

Medicare coverage for some or all of the care of pancreas transplant recipients in 2014 varied from 1.7% for children to 87.5% for recipients aged 65 years or older (Figure Econ 3). From transplant through the first year posttransplant, the average reimbursement for pancreas recipients with primary Medicare coverage in 2008-2013 was $95 thousand for Part A and $24 thousand for Part B (Table Econ 7). Pancreas recipients were rehospitalized more frequently than kidney recipients: 254 compared with 151 rehospitalizations per 100 years at risk in the first year posttransplant (Figure Econ 7, Figure Econ 2). Rehospitalizations for cardiovascular complications were somewhat less common for pancreas recipients than for kidney recipients, 11 vs. 13. However, other reasons for hospitalization were much more common among pancreas recipients. Pancreas recipients were more expensive than kidney recipients during both the first and second years posttransplant, totaling $120 thousand in the first year, compared with $83 thousand, and $33 thousand in the second year, compared with $24 thousand (Table Econ 7, Table Econ 2, Table Econ 8, Table Econ 3). This compares with Medicare expenditure of $49 thousand during the year preceding pancreas transplant, 149% of second-year posttransplant costs (Table Econ 6). The total Medicare expenditure for pancreas recipients in 2014 was $232 million. The average Medicare expenditure in 2014 for 8305 pancreas recipients was $30,056, 121% of the average cost of kidney recipients and third least expensive per patient of all solid organ transplants (Table Econ 9). Pancreas recipients accounted for 4.7% of all solid organ recipients covered by Medicare and 5.3% of all Medicare expenditures for solid organ transplant recipients in 2014.

Liver

Medicare coverage for some or all of the care of liver transplant recipients in 2014 varied from 1.5% for children to 90.3% for recipients aged 65 years or older (Figure Econ 5). The average reimbursement for liver recipients with primary Medicare coverage from transplant through the first year posttransplant was $162 thousand for Part A and $26 thousand for Part B (Table Econ 12), totaling $188 thousand. This is more than twice the average Medicare expenditure for kidney transplant recipients, at $83 thousand (Table Econ 2). However, the long-term costs of care, represented by costs in the second year posttransplant, were similar for liver ($29 thousand) and kidney ($24 thousand) recipients (Table Econ 13, Table Econ 3, respectively). This compares with Medicare expenditure of $50 thousand during the year preceding liver transplant, 174% of second-year posttransplant costs (Table Econ 11). The total Medicare expenditure for liver recipients in 2014 was $739 million (Table Econ 14). Average Medicare expenditure in 2014 for 30,641 liver recipients was $25,992, 104% of the average cost for kidney recipients and second least expensive per patient of all solid organ transplants (Table Econ 15). Liver recipients accounted for 17.2% of all solid organ recipients covered by Medicare and 16.8% of all Medicare expenditures for solid organ transplant recipients in 2014.

Intestine

Medicare coverage for some or all of the care of intestine transplant recipients in 2014 varied from 0.7% for children to 84.6% for recipients aged 65 years or older (Figure Econ 7). Intestine transplant was the most expensive solid organ transplant reimbursed by Medicare in 2014, with average first-year costs of $349 thousand for Part A and $44 thousand for Part B (Table Econ 17), totaling $393 thousand. Intestine recipients were rehospitalized frequently, with 709 rehospitalizations per 100 years at risk in the first posttransplant year and 355 in the second (Figure Econ 8). Medicare expenditure during the second year posttransplant averaged $108 thousand compared with $130 thousand during the year preceding intestine transplant, 120% of second-year posttransplant costs (Table Econ 18, Table Econ 16, respectively). Because of the small number of procedures, the total Medicare expenditure for intestine transplant recipients in 2014 was $13 million (Table Econ 19), less than 1% of all Medicare expenditures for solid organ transplant recipients. However, at the patient level, average expenditure for 270 intestine recipients in 2014 was $55,979, 225% of the average cost of kidney recipients and the most expensive per patient of all solid organ transplants (Table Econ 20).

Heart

Medicare coverage for some or all of the care of heart transplant recipients in 2014 varied from 0.1% for children to 91.4% for recipients aged 65 years or older (Figure Econ 9). The average Medicare reimbursement from transplant through the first year posttransplant was $270 thousand for Part A and $35 thousand for Part B (Table Econ 22), totaling $305 thousand, the second-highest average total costs, below intestine transplant. Rehospitalization rates following heart transplant were similar to rates following kidney transplant, with 169 rehospitalizations per 100 years at risk in the first posttransplant year and 83 in the second (Figure Econ 10). However, average second-year Medicare costs after heart transplant, at $36 thousand, were 149% of second-year costs following kidney transplant (Table Econ 23). This compares with Medicare expenditure of $112 thousand during the year preceding heart transplant, 312% of second-year posttransplant costs (Table Econ 21). The total Medicare expenditure for heart recipients in 2014 was $485 million (Table Econ 24). Average Medicare expenditure in 2014 for 14,130 heart recipients was $37,174, 149% of the average cost of kidney recipients and the third most expensive per patient of all solid organ transplants (Table Econ 25). Heart recipients accounted for 8.0% of all solid organ recipients covered by Medicare and 11.0% of all Medicare expenditures for solid organ transplant recipients in 2014.

Lung

Medicare coverage for some or all of the care of lung transplant recipients in 2014 varied from 0.0% for children to 93.3% for recipients aged 65 years or older (Figure Econ 11). The average Medicare reimbursement from transplant through the first year posttransplant was $212 thousand for Part A and $28 thousand for Part B (Table Econ 27), totaling $240 thousand, the third-highest average total costs, below intestine and heart transplants. Rehospitalization rates following lung transplant were similar to rates following pancreas transplant, with 259 rehospitalizations per 100 years at risk in the first posttransplant year and 148 in the second (Figure Econ 12). However, average second-year Medicare costs after lung transplant, at $52 thousand, were 158% of second-year costs following pancreas transplant (Table Econ 28). This compares with Medicare expenditure of $32 thousand during the year preceding lung transplant, 62% of second-year posttransplant costs (Table Econ 26). The total Medicare expenditure for lung recipients in 2014 was $308 million (Table Econ 29). The average Medicare expenditure in 2014 for 7375 lung recipients was $47,615, 191% of the average cost of kidney recipients and the second most expensive per patient of all solid organ transplants (Table Econ 30). Lung recipients accounted for 4.2% of all solid organ recipients covered by Medicare and 7.0% of all Medicare expenditures for solid organ transplant recipients in 2014.

Pretransplant Costs

In this Annual Data Report, SRTR continues to release new data suitable for use in cost-effectiveness modeling in solid organ transplantation. The release of pretransplant data for lung last year has been expanded to include all organs this year. Future Annual Data Reports will continue to expand upon the depth and breadth of economic data reported to support inquiries of researchers, policy makers, and the general public.

Pretransplant costs of care for Medicare-covered transplant candidates who underwent transplant were summed across 30, 90, and 365 days before transplant. These are included for each organ for all recipients and stratified by age, sex, race/ethnicity, and organ-specific primary diagnosis. The cost of the inpatient hospitalization for transplant was included in the first-year posttransplant hospitalization costs, not in pretransplant costs. Including pretransplant costs facilitates detailed cost analysis. Pretransplant cost information may allow analysis of alternative treatment strategies, such as estimation of the adjusted cost of alternative therapeutic options to transplant.

Costs during the pretransplant year compared favorably to, i.e., were greater than, long-term posttransplant costs (represented by costs during the second posttransplant year) in most cases. This has long been understood with regard to kidney transplant, and is a fundamental pattern behind the argument that kidney transplant is cost saving. This pattern occurred for all other organs except for lung, and for many lung subsets. This pattern does not guarantee that all of these transplants are cost saving; cost savings requires relatively long survival at high costs without transplant, as with dialysis. However, cost savings from transplant are possible and cost effectiveness is highly likely in selected cases other than kidney transplant.

The reported pretransplant costs create the opportunity to consider the relative costs and, with assumptions concerning pretransplant and posttransplant survival, the cost-effectiveness of solid organ transplant modalities compared with alternative treatments. Non-renal solid organ transplants may be cost-saving for certain groups of patients. Cost-effectiveness of many, and perhaps most, solid organ transplants is likely.
